چکیده

AN apparently haphazard distribution of ventricular extrasystoles may not be so haphazard as it appears and indeed may mask an underlying regularity. We previously reported a case in which careful analysis uncovered a simple but remarkable phenomenon, namely, that the interectopic intervals always contained an odd number of sinus beats.1 This was explicable on the assumption that a bigeminal rhythm persisted in a concealed form. Since then we have observed this and related phenomena on many occasions. Our purpose here is to present the concept of "concealed extrasystoles," which has not been previously reported except for brief mention in our earlier communication.

Concealed ventricular extrasystoles due to interference and due to exit block.

Two cases of sinus rhythm with ventricular extrasystoles are reported in which extrasystoles arising from the same focus obey the rule of "comcealed bigeminy" and the "rule of bigeminy". In a comparatively rapid sinus rhythm, shortening of the sinus cycle favors the appearance of extrasystoles, and the extrasystoles obey the fule of "concealed bigeminy"; namely, sinus impulses intervene between...

Ventricular quadrigeminy as a manifestation of concealed bigeminy.

Long rhythm strips were analyzed from five patients with frequent ventricular extrasystoles. The predominant pattern was quadrigeminal; i.e., three sinus beats between extrasystoles. However, about 20% of the interectopic intervals contained numbers of sinus beats (S) greater than three. Analysis of the distribution of such values of S greater than 3 revealed that there were many more odd than ...

Mechanisms of ventricular parasystole.

Eight cases of ventricular parasystole are reported. In all these cases, regardless of whether parasystole seems intermittent or "continuous," the presence of second degree entrance block of the Mobitz type I was suggested. Parasystole alternated with concealed extrasystolic bigeminy showing occasional reentrant extrasystoles. Such intermittent parasystole appears to originate in the reentrant ...
